Rolling out AI code review without getting burned: the checklist

Three rules from teams that switched AI code review on safely, and the checklist to run before, during and after you turn it on.

AI reviewers are good at reading diffs. They are not good at ignoring politely worded instructions, and everything they read was written by the person whose change they are judging.

The teams that rolled AI review out without incident kept three rules. The AI informs; a rule decides. Everything in a pull request is untrusted. Nothing goes live before it has proven itself next to your engineers. Here is the full checklist behind them.

  1. Before you switch it on: the AI informs, a rule decides

Decide what the reviewer is allowed to do before it does anything.

Rewind’s version of this is “keeping the judgment (AI) and the gate (deterministic code) separate.” The model gives findings; a small, tested piece of ordinary code turns them into a decision.

  2. Treat everything in a pull request as untrusted

Anything the reviewer reads can carry instructions, and the author of the pull request controls all of it.

Stop and ask if you see

    • Hidden text works. In July 2026 Manifold Security showed hidden HTML comments in a pull request description being followed by an AI reviewing agent: the proof of concept approved the change, fetched a confidential wiki page and posted it back as a comment, and told the agent not to mention it to the human reviewer.
    • So do issues. Trail of Bits showed an instruction planted in an issue leading a coding agent to slip a backdoor into its own pull request.
  3. Run it in shadow mode before you trust it

Let it review next to your engineers, with no power, until it has earned some.

  4. After it’s live: set expectations and keep the evidence

The short version

Let AI review the code. Don’t let it decide what ships.
